Caught between locomotive and car. Man found crushed under car, presumed he tried to jump across between cars and slipped down. Man was on a car, going 30 miles an hour, with other laborers and his hat blew off. In trying to recover it he fractured skull, causing death. While attempting to cross railroad track was struck by a train. Crushed between two railroad cars being moved for coupling. Man was carrying lumber across railroad track and was struck by an engine. Man struck by train and killed. While driving dump cart across railroad tracks cart was struck by freight car, killing driver. Deceased was walking along the tracks when struck by an express. - - Deceased was drawing truck along platform and pulled same in front of approaching engine. Man riding on freight car to go to work at furnace. He was standing on step of car and when car reached iron wharf he was caught between edge of wharf and car and crushed to death. Two men were pushing an empty railroad car by hand, along a construction track. One man slipped or stum- bled, falling in front of car wheels. To save himself he rolled off the rail under the car and was caught by a break beam or other projection, breaking back. Stepped between two engines to uncouple them and was squeezed between them. While cutting the air hose between two cars he did not wait for engine to push back to make second coupling, and was caught between sills of cars fracturing skull. Man was working at a lathe driven by an electric motor, and on touching the lathe he received an electric shock— 230 volts. Man came in contact with charged electric light wire on pole, while putting rings on messenger strand. Feed wire fell during storm, leaving one end on ground and the other dangling from cross arm, deceased took hold of end dangling from cross arm and re- ceived shock causing death. Hand-power freight elevator; elevator broke, man and elevator falling down shaft together. Found with head crushed, was running elevator. Stepped on elevator safety doors, caught in elevator hatchway. Employee pulled hoisting hand cable instead of the one to go down. Elevator being at top was automatically shut off; continued pulling caused safety collar to loosen and elevator to ascend; the top of cage jammed against roof, a cable broke, and casting on end of other gave way, elevator dropping to basement, a drop of about one and a half floors. Bucket and chain elevator: thinking job completed tackle was taken down. Elevator moved slowly at first, but when weight of chain and bucket got over centre it moved so rapidly that man was unable to get away and the end of the chain struck him. After elevator had started down, man went to elevator shaft and leaned over, top of elevator caught him while in this position and crushed his face. Opened door leading into- elevator shaft, and fell into shaft and was killed. Was climbing on elevator while same was going up; was caught across the abdomen, between elevator platform and shaft. * While passing under elevator of movable type, platform, on which was a barrel of varnish, about 540 lbs., fell. Slipped in reaching from ladder and attempting to balance himself from pillar to roof. Walking down ladder, missed his footing and fell about twenty-five feet, striking his head on two beams in falling and landing in a lot of iron beams. Wall of building had been built about thirty feet high. Starting on new scaffold which man had built one foot eight, inches high, outside of wall, pushed it over and fell with if. Man and partner were holding hatch cover, partner let go of his end, the deceased tried to hold his end alone but he slipped and fell through hole head first. The injured was not at work at the time but was walk- ing around and fell down and broke his leg. Prob- ably due to the infirmities of old age. Man was removing steel roofing sheets from wire mill. There was a scafold below him but the roofing sheet that fell with him tipped him off. Deceased was sitting on plank with legs astride of beam when plank slipped back as he started to use rivet gun. His pressure on gun caused plank to slip, caus- ing him to lose his balance and fall to first floor, about forty feet. Walking on platform two feet wide over rear end of vats, containing boracic acid liquor, and in stepping from one platform to another (opening two feet), on same level, he slipped and fell into a vat of hot liquor. Considerable mist or vapor around vat caused by cold air entering through window and striking hot liquor. In attempting to cut time and go home early deceased crossed tanks filled with hot soda liquor and in so doing fell into the tank and was scalded fatally.
