Revisions cause problems for oncologists, epidemiologists and statisticians who are interested in studying trends in cancer mortality and morbidity. Data must be converted from one code $0 another in order to achieve comparability. Although a conversion (1) was already published converting the neoplasms (Chapter II) of the International Classification of Diseases, 1965, Eighth Revision (ICD-8) (2) and the 8th revision, International Classification of Diseases, Adapted (ICDA-8) (3) to the Neoplasms (Chapter II) of the International Classification of Diseases, 1975, Ninth Revision (ICD-9) (4), this conversion can not be used when one wants to convert in the opposite direction, i.e. from ICD-9 to ICD(A)-8.% Whereas the first conversion involved expansion, going in the opposite direction involves collapsing, i.e. adding several codes together to be equivalent to a category in ICD-8. Since only a few years of data as yet have been coded by ICD-9, it is easier to convert a smaller data set to ICD(A)-8 categories, for which 10 years of material are already available. The layout of this conversion has been kept similar to the first (1) conversion mentioned with Part I showing the principal 3 digit changes, Part II the detailed conversion and Part III a table for computer conversion. A computer program for this conversion exists at the National Cancer Institute, and is available on request. The detailed conversion in Part II shows the ICD-9 terms and categories on the left and the exact detailed equivalent category of ICD(A)-8 on the right, as well as the direct computer conversion category of ICD(A)-8 in a separate column. In nearly all cases the category is exactly the same in both these last two columns. A typical example of "collapsing" is shown under stomach, where categories 151.3 (fundus), 151.4 (body), 151.5 (lesser curvature, NOS), 151.6 (greater curvature, NOS) and 151.8 (other) in ICD-9 all collapse or are converted back to 151.8 (other specified parts) in ICD(A)-8. To make it easier for computer programmers a table is given in Part III giving only the first column from Part II, the category from ICD-9, and the direct computer conversion column of ICD(A)-8. No terms are given in this table. ¥Note: ICDA-8(3) is an adapted version of the World Health Organization's (WHO) ICD-8(2), published and used in the United States. There are very few differences between these two 8th revisions for neoplasms but those that exist are marked with an (¥). The symbol ICD(A)-8 has been used to refer to both manuals. A new concept introduced in ICD-9 was categories (ending in a 4th digit of .8) which include a single lesion crossing the boundaries of two or more fourth digit categories. Since this concept did not exist in ICD(A)-8, most .8's in ICD-9 have been converted either to the old .8 meaning "other specific parts" or to .9 meaning "unspecified." The major differences occuring when using ICD-9 rather than ICD(A)-8 have already been discussed (page ii of ref. (1)). It should be pointed out that several diseases are now considered neoplasms that were not in Chapter II (Neoplasms) of ICD(A)-8. These diseases include Sezary's disease (202.2) in ICD-9, malignant histiocytosis (202.3) and Letterer = Siuwe disease (202.5). Although there is no problem in converting these to categories in ICD(A)-8, it was impossible to identify them separately during the 8th revision as most of these diseases were part of a "catch all" category - 279 - (other and unspecified metabolic diseases). However, the total number for malignant neoplasms in ICD-9 could be adjusted to exclude these 3 subcategories in comparing a total figure to a data set coded by ICD(A)-8. There are a few categories that were considered malignant during the 8th revision that are now considered only neoplasms of uncertain behavior: polycythemia vera, myelofibrosis and plasmacytoma. These diseases represent a considerably higher frequency than those added under the 202 category. It is important to mention, especially for morbidity, the addition in ICD-9 of categories 230 - 234 for carcinomas in situ. These have all been converted to categories of unspecified nature in the 8th revisions. The biggest problem in converting from ICD-9 to ICD(A)-8 concerns the peripheral nerves and sympathetic nervous system which were coded to specific 4 digit categories 192.4 and 192.5 respectively in the 8th revisions but are scattered throughout category 171._ (malignant neoplasms of connective and other soft tissues) in ICD-9. These cases can not be converted from ICD-9 to ICD(A)-8. Fortunately this does not represent many cases. ¥¥ refers to this paragraph. For those in the United States desiring a conversion from ICD-9-CM(5) to ICD(A)-8, they can use Table 1 of reference (6) (Differences between Chapter II, Neoplasms of ICD-9 and ICD-9-CM) to convert to ICD-9 and then use this conversion to get to ICD(A)-8.
