Since this new edition was planned as a major revision, there is a 15-year span, from 1977- 1992. Oncologists and epidemiologists often are interested in studying trends in cancer incidence and mortality, so it is imperative that they be able to compare data with codes from difierent revisions of the ICD’s. This conversion was developed to convert or compare the Neoplasm Section (chapter 11) of the International Classification of Diseases, 1975, Ninth Revision (ICD—9)‘ and the International Classification of Diseases, 9th Revision, Clinical Modification, (ICD-9-CM)a to the International Statistical Classification of Diseases and Related Health Problems, (ICD-10)f The ICD-lO has increased the specificity and detail for coding many sites of origin of cancers. However, this specificity cannot be achieved for old records without reviewing and recoding them. In fact, some conditions or codes did not exist or were not recognized in ICD-9 or in ICD-9-CM, which are commonly referred to as ICD-9-(CM). As can be seen from the page layout in part I of this conversion, the left hand side of each page has the categories and terms from chapter II of [CD-9, volume 1 (numeric index) and when they don’t exist in ICD-9, terms from ICD-9-CM are also included. Part I begins with the malignant neoplasm category 140._ (malignant neoplasm of lip). On the right side of the page is the equivalent category or categories in the new ICD-lO. There are two columns for the categories in ICD-lO. One is "Direct Computer Conversion'" the other is "ICD-lO Specific Code." As has been mentioned above, many site categories have been divided into more detail in ICD-lO. For example, 182.0 (malignant neoplasm of the corpus uteri, except isthmus) in ICD-9 has now been divided in ICD-lO into C541 (Endometrium), C542 (Myometrium), C543 (Fundus uteri) and C549 (Corpus uteri, unspecified). These terms and categories are shown in the column headed "ICD-lO Specific Code." Inasmuch as most users have neither the ability nor the time to review records, the "Direct Computer Conversion" column gives C549 (Corpus uteri, unspecified) as the category for conversion in ICD-lO. A computer conversion can be made with the categories suggested in the "Direct Computer Conversion" column. Part II is an aid to such programming for conversion of ICD-9 and ICD-9-CM to ICD-10: it includes only the ICD-9 and ICD-9-CM categories and the direct conversion column of part I, which shows the equivalent ICD-10 code categories. No terms are given in part II. An attempt has been made to convert 3-digit terms of ICD-9-(CM), but it is sometimes difficult to do this and a very unspecific term must be resorted to. If someone does use only 3-digit terms, these cases would have to be hand-reviewed. There are a few 4-digit terms, mostly in the .8 category (overlapping) that are also impossible or difficult to convert. Since they are in the minority, they could be hand-reviewed. The traditional ICD structure has been retained, but an alphanumeric coding scheme replaces the previous numeric one. This provides a larger coding frame and leaves room for future revision without disruption of the numbering system. Chapter II, "N eoplasms" has been subjected to field-testing although the changes in its content have been of a minor nature. The editors of ICD-10 tried to keep changes at the 3-character level of the malignant neoplasm section to a minimum. However, some changes were necessary. Three new categories have been added, namely, Mesothelioma (C45) and Kaposi’s sarcoma (C46) and independent (primary) multiple sites (C97). When 3-digit categories have been split into two, the non-specific (NOS) category has been put into the second one. Changes at the 3-character level include the following: - Malignant neoplasm of tongue (141) was expanded to include a 3-character category for base of tongue (C01) and one for other and unspecified parts of tongue (C02). 0 Maior salivary glands (142) was similarly divided into parotid gland (C07) and other and unspecified major salivary glands (C08). . Other and unspecified parts of mouth (145) was changed to comprise malignant neoplasm of palate (C05) and other and unspecified parts of mouth (C06). - Oropharynx (146) was expanded into tonsil (CO9) and oropharynx (C 10). ii Hypopharmg (148) was changed to pyriform sinus (C12) and hypopharynx (C13). Malignant neoplasms of rectum, rectosigmoid iunction and anus (154) was distributed across three 3-character codes, namely, rectosigmoid junction (C 19), rectum (020) and anus and anal canal (021). Gallbladder and extrahepatic bile ducts (156) was expanded into gallbladder (C23) and other and unspecified parts of biliary tract (C24). Nasal cavities, middle ear and accessory sinuses (160) was divided into nasal cavity and middle ear (C30) and accessory sinuses (C31). Trachea, bronchus and lung (162) became two separate categories of trachea (C33) and bronchus and lung (C34). Thymus, heart and mediastinum (164) were distributed into thymus (C37) and heart, mediastinum and pleura (C38). Bones and articular cartilage (170) has been expanded to two categories of limbs (C40) and of other and unspecified sites (C41). Connective and other soft tissue (171). In ICD-9 the category of peripheral nerves, sympathetic, and parasympathetic nervous system was coded to connective tissue. In ICD-10 there are 3-character categories for peripheral nerves and autonomic nervous systems (C47) and other connective and soft tissue (C49). Ovary and other uterine adnexa (183) was changed to include a 3-character category for ovary (C56) and another for other and unspecified female genital organs (C57). Other and unspecified female genital organs (184) was further divided as to vulva (C51) and vagina (C52). iii - Penis and other male genital organs (187) was changed to two categories, namely, penis (C60) and other and unspecified male genital organs (C63). - Kidney and other and unspecified urinary organs (189) has been expanded into four categories for kidney, except renal pelvis (C64), renal pelvis (C65), ureter (C66) and other and unspecified urinary organs (C68). . . Other and unspecified parts of nervous system (192) was changed into one category for meninges (C70) and another for malignant neoplasms of spinal cord, cranial nerves and other parts of central nervous system (C72). - Other endocrine glands and related structures (194) was divided into a specific category for adrenal gland (C74) and a separate one for other endocrine glands and related structures (C75). . Qmphoid. hematopoietic and related tissue (200-208) were defined with two different divisions: a) Lymphosarcoma and reticulosarcoma (200), Hodgkin’s disease (201) and other malignant neoplasms of lymphoid and histiocvtic tissue (202) were organized into a 3-character category for Hodgkin’s disease (C81), one for follicular (nodular) non-Hodgkin’s lymphoma (C82), one for diffuse non- Hodgkin’s lymphoma (C83), one for peripheral and cutaneous T-cell lymphomas (C84) and one for other unspecified types of non-Hodgkin’s lymphoma (C85). b) Multiple myeloma and immunoproliferative neoplasms (203) was divided into malignant immunoproliferative diseases (C88) and multiple myeloma and malignant plasma cell neoplasms (C90). A complete list will be found in Part I. A few conditions that were not considered neoplastic during the use of ICD-9 and ICD-9-CM are now in the neoplasm section of ICD-10. These were all coded in other chapters of ICD-9. An example is Waldenstrom’s macroglobulinemia, which was 273.3 in Chapter 3 of ICD-9-(CM) which included "Immunity disorders", is now iv under Neoplasms C88.0 under "Malignant immunoproliferative diseases." There would be no way to convert any of these to ICD-lO categories in the malignant neoplasm section without reviewing records. An ANSI COBOL computer program has been developed to effect this conversion. The program will be made available upon request.
